It is the quiet fear behind almost every "am I too old for this" question I get: not the climb, the knees. Specifically, the long way back down. So here is the straight answer. Why the way down is the dangerous part
Going up is concentric and aerobic. It taxes your engine. Going down is eccentric: your quads act as brakes, lengthening under load, thousands of times per descent. That braking is what irritates knees, and a body that only trained uphill and flat has never asked its legs to do it at volume. Add tired legs on day ten of a trip and you have the classic recipe for a knee that starts talking to you halfway down.
Train downhill early. The knees do not get hurt on the mountain. They get hurt because nobody prepared them for the mountain.
The training that actually protects them
- Single-leg strength. Split squats, step-ups, and Bulgarian split squats build the exact pattern the trail uses, one leg at a time.
- Eccentric and step-down work. Slow, controlled step-downs teach your quads to brake under load. This is the single most specific thing you can do for descents.
- Actual downhill. If you have hills, walk down them on purpose, building volume gradually. If you do not, a treadmill and a stair machine get you most of the way.
- Time. Connective tissue adapts slower than muscle, and slower again over 45. Give it a long runway. Shaky knees a year out are a training project. Shaky knees a month out are a problem.

Use trekking poles, especially on descents. They offload real weight from the knees on the way down, which is where the damage accumulates. And control your speed. Shorter strides, slower pace, and letting your legs brake gently beats bombing down and paying for it the next morning.

Training soreness is one thing. Sharp, localized, or persistent joint pain is another, and it is not something to coach through or push past.
